North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
1
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The names of the owners of the property against which this lien is filed are Oscar J. Thoel, Richard
T. Thoel and Christine E. Thoel, 5 Clearview Avenue, Carlisle, Pennsylvania 17013.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 5 Clearview Avenue, Carlisle, Pennsylvania.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid water and sewer user fees against the above-described property in the
amount of $680.23, plus any additional fees incurred thereafter.

Interest is claimed on the above-mentioned fees until paid in full.

by the same South 77 degrees 20 minutes East 24.69 feet to a point; thence by the line of lot no.
19 on said plan, nor or formerly of Lee R. Bear, et ux., South 12 degrees 30 minutes West 150 feet
to a point at the Northem Line of Bellaire Avenue; thence by the Northem line of Bellaire Avenue
North 77 degrees 20 minutes West 61..0 feet to a point; thence by a curve to the right having a
radius of 80 feet, the arc distance of 76.56 feet to a point; thence by the Eastern line of Clearview
Avenue North 22 degrees 30 minutes West 136.31 feet to a paint, the Place of BEGINNING.
BEING Lots Nos. 17 and 18 on the plan of lots known as Hill Top Manor, duly recorded in
Plan Book 11, Page 39.
BEING THE SAME PREMISES which Deutsche Bank National Trust Company, as
Indenture Trustee for American Home Mortgage Investment Trust 2005-2, Mortgage-Backet Notes,
Series 2005-2 by American Home Mortgage Servicing, Inc., its Attorney in Fact by a Power of
Attorney recorded 6/25/10 as Instrument #201016907, by its Deed dated October 29, 2010, and
recorded simultaneously herewith, in the Office of the Recorder of Deeds in and for Cumberland
County, at Carlisle, Pennsylvania, to Instrument Number ,granted
and conveyed to Oscar J. Thoei, the Grantor herein.
