North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM

The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The names of the owners of the property against which this lien is filed are Douglas B. Thompson
and Carolyn D. Thompson, 2709 Shire Lane, Rocky Mount, North Carolina 27804.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 140 Partridge Circle, Carlisle, Pennsylvania.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid water and sewer user fees against the above-described property in the
amount of $616.48, plus any additional fees incurred thereafter.

Interest is claimed on the above-mentioned fees until paid in full.

Pennsylvania in Plan Book S8, Page 100, more particularly bounded and described as follows,

BEGINNING at a point on the eastern right-of--way line of Partridge Circle formerly known as
Hemlock Drive) at the northeastern corner of Lot No. 12-B on the aforementioned Plan of Lots;
thence along the eastern right-of--way line of Partridge Circle (formerly known as Hemlock
Drive), North 3S degrees 14 minutes 08 seconds East, a distance of 37.99 feet to a point; thence
by a curve to the right having a radius of 12 feet, an arc length of 17.88 feet to a point on the
southern right-of--way line of Partridge Circle; thence along the southern right-of--way line of
Partridge Circle, South S9 degrees 24 minutes 23 seconds East a distance of 114.35 feet to a

BEING the same premises which Albert C. Kuhn and Edith B. Kuhn, his wife, by Deed dated
March 20, 2001 and recorded March 22, 2001 in the Office of Recorder of Deeds in and for
Cumberland County, Pennsylvania, in Deed Book 241 Page328, granted and conveyed unto
Douglas B. Thompson, Grantor herein.
AND the said Grantor hereby covenants and agrees that he will warrant SPECIALLY the
property hereby conveyed.
This conveyance is between Husband and Wife and is therefore Tax Exempt.
