2. THOMAS STRAUSBAUGH, natural father of herein named minor, is an adult individual who currently
resides at 339 15th St, New Cumberland, Pa 17070.
3. Nicolas Matthew Emig, minor and natural child of Laura Emigand Thomas Strausbaugh, currently
resides at 1006 W Foxcroft St, Camp Hill Pa 17011.
Thomas Edward Strausbaugh has partial custody of Nicolas Matthew Emig. Thomas Edward
Strausbaugh wishes to give full custody of Nicolas Matthew Emig to Laura Beth Emig. Laura Beth
Emig will have full legal and physical custody of Nicolas Matthew Emig, including the dght and
responsibility to make decisions regarding Nicolas Matthew Emig's education, health, financial and
any other decision that may be required to propedy care for said minor.
5. Laura Emig wishes to accept full, legal and physical custody of Nicolas Matthew Emig.
