PETITION FOR CHANGE OF NAME
Petitioner, Parker Eli Dyson, a minor, by and through his guardians, John Arthur Milner
and Abigail Milner, ("Petitioner"), by and through counsel, Tucker Arensberg, P.C., respectfully
request the Court to giant a change of name for Parker Eli Dyson, a minor, and in support thereof
aver as follows:
1. Petitioner, Parker Eli Dyson, is a minor child. Abigail Milner is Petitioner's
natural mother, and Jahn Arthur Milner is Mrs. Milner's husband.
2. Pursuant to a June 12, 2012 Order of this Court at Civil Action No. 2007-3713,
Mr. and Mrs. Milner have primary physical custody of Petitioner.
3. Mr. and Mrs. Milner share legal custody of Petitioner with Petitioner's natural
father, Terry L. Jones, a resident of Cumberland County, Pennsylvania.
4. The June 12, 2012 Order of this Court at Civil Action No. 2007-3713 also
provides for period;. of partial physical custody of Petitioner to Petitioner's maternal
grandparents, David Dyson, Jr. and Jill S. Dyson, who reside in Cumberland County,
Pennsylvania.

5. Petitioner currently resides at 10412 Redbud Drive, Reminderville, Ohio 44202,
as permitted by the June 12, 2012 Order of this Court at Civil Action No. 2007-3713..
6. Petitioner formerly resided in Cumberland County, Pennsylvania at 17 Sunfire
Avenue, Camp Hill, Pennsylvania, 17011.
7. Notwithstanding Petitioner's current residence in Ohio, this Court has jurisdiction
to hear this Petition and enter an Order changing Petitioner's name.
8. Petitioner and his guardians desire to change Petitioner's name to Parker Eli
Milner, to reflect the status of Petitioner and Mr. and Mrs. Milner as a family unit.
9. The change of Petitioner's name to Parker Eli Milner is in the best interest of
Petitioner.
10. The change of Petitioner's name to Parker Eli Milner will not affect the rights of
Petitioner's natural father or maternal grandparents.
11. Notice of this Petition shall be served upon Petitioner's natural father, Terry L.
Jones, and. Petitioner's maternal grandparents, David Dyson, Jr. and Jill S. Dyson., pursuant to
the June 1'2, 2012 Order of this Court.
12. There are no judgments or decrees of like character of record against Petitioner.
13. Petitioner has not been convicted of any crime.
14. This Petition is not filed to defraud creditors.
Wl-IEREFORE, Petitioner, Parker Eli Dyson, a minor, by and through his guardians, John
Arthur Milner and Abigail Milner, requests this Honorable Court:, pursuant to the Act of
December 16, 1982, P.L. 1309, No. 295, § 2 (as amended), 54 Pa.C.S.A. § 702, for the
following:

To direct that notice be given on this Petition and hearing; and
