PETITION FOR CHANGE OF NAME
AND NOW, comes Josepha Anne Wade, by and through her counsel, Julie A.
Wehnert and petitions this Honorable court as follows:
1. Petitioner, Josepha Anne Wade (nee Dolinar), dob July 14, 1953, wishes
to have her name changed to JoAnn Wade.
2. Petitioner is requesting this name change because she has always gone
by the name JoAnn Wade in both her personal and professional life. JoAnn Wade is
what is listed on the Petitioner's social security card and what was on her driver's
license until Petitioner had to change it to match her newly issued passport.
3. Petitioner has life-long friends and colleagues that do not know her by any
other name other than JoAnn Wade.
4. Petitioner wishes to officially change the name on her birth certificate so
that all forms of identification, such as her passport, can be under JoAnn Wade.

5. The Petitioner has resided at 849 Mandy Lane, Camp Hill, Pennsylvania
17011 during the five years preceding the filing of this Petition.
6. Petitioner represents that there are no judgments or decrees of like
character of record against the Petitioner.
WHEREFORE, Petitioner requests the court to schedule a hearing on this
Petition and to enter a decree changing the Petitioner's name from Josepha Anne Wade
to JoAnn Wade.
