Y: ~...i,l r? ~" } s~ C°J The Petition of Linda S. Treese, Executrix of the Estate of Velma Oakes, respectfully states that: 1. Velma E. Oakes, ("Decedent") died on September 25, 2010, a resident of East Pennsboro Township, Cumberland County, Pennsylvania. 2. Petitioner, whose address is 785 Erford Road, Camp Hill, PA 17011, is the daughter of Decedent. 3. On the date of her death, no member of the family was a part of the Decedent's household. 4. Decedent was widowed and unmarried as of the date of her death. 5. Decedent left a Will, dated October 31, 2003, which was probated on December 28, 2010, and Letters of. Testamentary were granted to Petitioner by the Register of Wills of Cumberland County on December 28, 2010. 6. Decedent was survived by the following persons entitled to share from her Estate. There is no person who is entitled to the family exemption and the family exemption has not been claimed. 8. In addition to the assets listed in Paragraph 8, Decedent held a joint account at PNC Bank with her daughter, Linda S. Treese, which had a date of death balance of $1,215.99. From this account, New York Life was reimbursed an erroneously paid annuity payment of $133.24 resulting in a balance of $1,082.75. 10. Upon the death of Velma E. Oakes, Linda S. Treese became the owner of the PNC Account. Linda S. Treese made a loan of the monies to the Estate in the amount of the funds then in the PNC account, $1,215.99, to provide liquidity to the Estate. 12. After repayment of the erroneous deposit by New York Life in the amount of $133.24, Linda S. Treese is entitled to be repaid the sum of $1,082.75 13. The names of all claimants who are not paid in full of whom the Petitioner has notice or knowledge are: Commonwealth of Pennsylvania Department of Public Welfare Bureau of Program Integrity Division of Third Party Liability Estate Recovery Program Post Office Box 8486 Harrisburg, PA 17105-8486 Estate Recovery: $132,760.36 (A copy of Commonwealth's Claim is attached hereto). 14. On October 31, 2012, the Commonwealth of Pennsylvania Department of Public Welfare accepted the sum of $479.98 as payment of its existing claim. (A copy of the Department of Public Welfare's letter is attached hereto) 15. A Pennsylvania Inheritance Tax Return was filed with the Register of Wills on June 16, 2011. Inheritance tax was not due and a statement of "accepted as filed" was issued by the Pennsylvania Department of Revenue (a copy is attached hereto). 16.
