IN RE: PETITION FOR DETERMINATION OF INCAPACITY
AND APPOINTMENT OF GUARDIAN
ORDER OF COURT
AND NOW, this 28" day of December 2012, upon review of the Petition for
Determination of Incapacity and Appointment of Guardian, counsel is appointed to
represent the alleged incapacitated person, Rena G. Burk. The court appoints Lisa M.
Coyne, Esquire, to represent the alleged incapacitated person in these proceedings.
It is hereby ORDERED and DIRECTED that a hearing is scheduled for 25 March
2013 at 9:30 a.m. in Courtroom Number 6, of the Cumberland County Courthouse,
Carlisle, Pennsylvania. A CITATION shall be issued upon Rena G. Burk to show cause
why she should not be adjudicated an incapacitated person and have a guardian
appointed for her person and estate. PETITIONER shall serve Rena G. Burk with
notice of the hearing, in accordance with 20 P.S. 5511(a), no less than twenty (20) days
prior to the hearing.
Furthermore, the alleged incapacitated person shall be in attendance unless it
can be established by his physician that her mental or physical condition would be
harmed by her presence at the hearing. This may be established in the form of written
interrogatories.

PETITIONER shall properly serve notice of the petition and hearing upon all
persons entitled to share in the estate of the alleged incapacitated person, and proof of
said service shall be filed prior to the hearing.
