1. The above named decedent and his/her estate are indebted to Pennsylvania State Employees
Credit Union (PSECU) as hereinafter set forth.
2. The total amount of the aforesaid claim, namely, Share 4 Balance, $709.11 is now owing to
PSECU by the estate of the said Jennifer Helman, deceased, and no part of the same has been
paid.
