Petitioner's, Vijay S Teru and Rekha S Teru, requests that this Court order a c~i~fige~~f
name for Smaran Shankar Teru, minor child and in support thereof represent asaoll~s:
1. We request the Court to change the name of Smaran Shankar Teru ,minor child
date of birth is 10/26/1999 and is residing with us at the below address
2. Since 04/30/2006, we were residing at 476 Nursery Dr. N, Mechanicsburg, PA 17055.
3. There are no judgments or decrees pending against the petitioner's
4. We requests that our child's name be changed from Smaran Shankar Teru to
Smaran Sam Teru
5. The reasons for the requested change of name is since from the childhood, he goes
by name Sam in the school and family friends around, we would like to change his
current middle name Shankar to Sam. This is the interest of our child and the
petitioner's.
WHEREFORE, the petitioner's respectfully requests that this Court enter an Order
Changing his name of our minor child from Smaran Shankar Teru to Smaran Sam Teru

AND NOW, this 28th day of January 2013, upon consideration of the Petition for
Change of Name, a HEARING shall be held on 13 March 2013, at 3:00 p.m. in
Courtroom Number Six of the Cumberland County Courthouse, One Courthouse
Square, Carlisle, Pennsylvania.
NOTICE of the filing of the petition and of the date, time, and place of the above-
scheduled hearing shall be published at least TEN (10) DAYS prior to the hearing in two
newspapers of general circulation in Cumberland County or an adjoining county, one of
which may be the Cumberland County Law Journal, in accordance with 54 Pa. C.S.A. §
701 and C.C.R.P. 5. Proof of publication must be filed with the Cumberland County
Prothonotary prior to the above-scheduled hearing.
