JASON M. MOORS, Owners MECHANICS' LIEN CLAIM Madison Homes Group, Inc., by and through its undersigned counsel, files the following Mechanics' Lien Claim: 1. The Claimant is Madison Homes Group, Inc., a Pennsylvania corporation, which has a principal place of business at 405 Bridge Street, New Cumberland, Pennsylvania 17070. Claimant files this claim as a contractor. 2. The Owner of the real estate, which this claim is filed, is Jason M. Moore, adult individual, who resides at 455 Fickes Road, Dillsburg, Pennsylvania 17019. 3. The last day Claimant supplied material was December 8, 2012. 4. The party with whom Clamant contracted was Jason M. Moore, owner of the property. 5. A demand for payment and an informal notice of intent to file a Mechanics' Lien was sent to Jason M. Moore on December 7, 2012, to which no response has been received. A true and correct copy of the informal notice of intent to file a Mechanics' Lien is attached and made a part hereof as Exhibit A. nti~ ~'~ ~ ~ S6 ~ a ~( e ~~ ~~~ ~~a ~ygl5 5. Claimant supplied material and labor necessary for the improvement to the residence at 808 16th Street, New Cumberland, PA 17070, as fully described on the attached invoices made a part hereof as Exhibit B. 6. The prices charged for the material are as sated on the invoices attached as Exhibit B, which is the amount claimed to be due of $16,577.09. 7 The real property subject to the lien is situate in the Borough of New Cumberland, County of Cumberland, Pennsylvania, known as 808 16th Street, New Cumberland, Pennsylvania 17070. A Deed dated September 5, 2006, and recorded in Cumberland County on September 6, 2012, in Book 276 at Page 2667, a true and correct copy of which is attached and made a part hereof as Exhibit C. Said property is also identified with a Tax Parcel No.: 26-23- 0543-560. It is my understanding that George has attempted to get you to agree to sign a statement saying that his company will be paid out from the proceeds of the sale of the property, yet you have failed to return an executed statement. I am reseeding that statement for you to sign and return to me with regards to that matter.
