No: COMPLAINT Plaintiff State Farm Mutual Automobile Insurance Company is an insurance carrier licensed to conduct business in the Commonwealth of Pennsylvania and having as one of its principal places of business the above-captioned address. Virginia Yeingst is an adult, and on or about August 9, 2012, was the registered owner of a 2003 Chrysler motor vehicle and was insured under a policy of insurance issued by State Farm Mutual Automobile Insurance Company. 3. Defendant Bazbaza Monismith is an adult residing at 158 N West Street, Apt. 1 in Carlisle, Pennsylvania 17013, and on or about August 9, 2012, was the registered owner and operator of a 1996 Toyota motor vehicle. 4. On or about August 9, 2012, while operating the aforesaid Toyota motor vehicle in Cazlisle, Pennsylvania, Defendant Bazbaza Monismith disregazded a posted stop sign on North West Street and struck Plaintiffls insured's Chrysler motor vehicle, causing extensive property damage thereto in the amount of $5,814.28. 5. The aforesaid accident was a direct and proximate result of Defendant(s) negligence, cazelessness, and/or other liability-producing conduct. 6. Pursuant to the insurance policy herein mentioned, Plaintiff State Farm Mutual Automobile Insurance Company a/s/o Virginia Yeingst has incurred and paid expense and other damages in the amount of $5,814.28. WHEREFORE, Plaintiff State Farm Mutual Automobile Insurance Company a/s/o Virginia Yeingst demands judgment against the Defendant in the amount of $5,814.28, which includes the Plaintiff's insured's deductible, plus interest and costs of suit. --____.
