Eby, and Michael J. Eby, all parties in interest in the above-captioned matter, and entitled to
share in the estate of the alleged incapacitated person, with notice of (1) the Petition filed
November 29, 2012, to have David James Eby declared an incapacitated person, and (2)
the hearing scheduled on the said Petition by the Court.
