I, LINDA K. BURNETT, of 2714 Cedar Street, Mechanicsburg, Pennsylvania 17055,
hereby certify that on the 27th day of December, 2012, at approximately 7:00 p.m., I
personally served David James Eby, the alleged incapacitated person, with the Citation
dated December 6, 2012, and Order of Court dated December 5, 2012, as well as a copy of
the Petition filed November 29, 2012, to have David James Eby declared an incapacitated
person, in my home at 2714 Cedar Street, Mechanicsburg, Pennsylvania 17055, where
David James Eby also is a resident and continues to reside indefinitely. Furthermore, I
hereby certify that that on the above date and time, I explained the contents of the Citation
and Petition to David James Eby in language and terms he was likely to understand.
