~d ir~laremoNursing and Rehabilitation Center where she adjusted quickly to
her new surroundings.
On May 26, 2009 the Office of Aging was granted Permanent Guardian of Persona
Ms. Chronister had ambulation problems. Ms. Chronister was diagnosed with chronic
obstructive pulmonary disease, anemia, dementia, coronary artery disease, Depression,
peripheral vascular disease, arthritis, ambulatory dysfunction, GERD, congestive heart
failure, and arteriosclerotic disease.
On visits by Aging staff, Winnie seemed to be doing well. She was happy and laughed a
lot and played with her doll. In August of 2012 she was moved from the dementia unit to
the first floor. On November 15, 2012 we received a call from her social worker that she
was experiencing a serious decline. She had become non-responsive and appeared to
have had amini-stroke. When visited the next day she seemed to have greatly
improved. She was alert and ate about 75% of her meal. Then on December 9,2012
she experience another non-responsive condition. She was on oxygen and resting
comfortably. She passed away on December 12, 2012.
The next day we notified the Chaplain at Muncy Prision who was going to notify
Winnie's daughter, Kim, who is a resident there. We asked about her being able to
come to the funeral service. The Chaplain doubted she would be allowed due to the
level of her crime. Her sister in Michigan, Wanda Russell, was also notified and stated
she would call her other sister in Florida.
The Agency had made pre-arrangements for the funeral. In addition, a pastor who had
visited her in her private home and then later in the nursing home conducted the
funeral.
ESTATE
Ms. Chronister had a guardianship account at Wells Fargo Bank. Her monthly income
was $967.00 from social security. Each month her expenses included $822.00 to the
EMAIL US AT' aging@ccpa.net OR VISIT OUR WF;BSiTE AT www.ccpa.net/aging
nursing home, $100 to the Office of Aging for the guardianship fee and $7.80 for an
insurance premium.
Attached is a summary of her expenses and income as well as a copy of the check
register. The final balance in her account is $3249.06. The Estate Recovery Office of
the Department of Public Welfare will be notified of her death.
