REPORT OF ACCOUNT CREATION
Pursuant to the Order of Court dated October 31, 2012, the undersigned reports the
creation of an inherited tax-deferred retirement account with Wells Fargo Advisors, Inc., on
December 6, 2012, consisting of the minor's share of Adamadia Deforest's IRA account, also
with Wells Fargo Advisors, lnc. The initial deposit to this account is $45,563.33.
Two inherited tax-referred retirement accounts have also been created with TIAA-CREF
on January 2, 2013, the first consisting of the minor's share of Adamadia Deforest's 401(A)
plan; and the other consisting of the minor's share of Adamadia Deforest's two 403(B) plans, all
with TIAA-CREF. The initial deposit to the first account is $9,085.33; and the initial deposit to
the second account is $119,506.39.
Two checking accounts with Metro Bank (one to hold principal and one to hold income)
were created 1'or the minor by the Co-Guardians on December 22, 2012. A nominal, initial $1.00
deposit was made to each account. The funds deposited did not belong to the minor, and it is
anticipated that these funds will be withdrawn as deposits of the minor's money are made.
