Rock Road, Newville, Cumberland County, Pennsldvania _ 1998-2000. 3. There are no judgments or decrees of like character of record against the petitioner, certification of an official search is attached hereto as Exhibit "A'. 4. Petitioner requests that this name be changed from Raymond Branch Call to Raymond Branch Gummings. S. The masons for the requested change of name are the following: (a) Petitioner has discomfort with his last name being the same word as a verb and a noun used in common speech; and (b) Petitioner wishes to change his last name based on a personal religious belief.
