Petitioner Bobbi Kaxe Aliff ,requests that this Court order a change of the
petitioner's name, pursuant to 54 Pa. Cons. Stat. Section 701, et sec ., and in support thereof

5. The reasons for the requested change of name are
Currently have my ex husband's last name. Do not want my maiden last name, would like
to make my middle name my last name.
6. The Petitioner has not been convicted of a felony.
WHEREFORE, the petitioner respectfully requests that this Court enter an Order
changing his/her name from Bobbi Kave Aliff to Bobbi Kaye
