PETITION OF YOUNG-UN COBY HUANG, A MINOR, BY AND THROUGH HIS
PARENTS, KUANG CHENG HUANG (FATHER) AND STELLA HUANG (MOTHER)
FOR CHANGE OF CHILD'S NAME TO COBY YONG AN HUANG
1. Your Petitioners are Kuang Cheng Huang and Stella Huang, adult individuals,
married to each other and residing at 707 Westwood Drive, Enola, Cumberland County,
Pennsylvania.
2. The subject of this Petition is your Petitioners' son, now known as Young-Un Coby
Huang; the child was born on September 7, 2004. A certified copy of the child's birth
certificate is attached hereto, marked Exhibit "A" and incorporated herein by reference.
3. The child has lived with his parents, the Petitioners, for all of his life; furthermore,
the child has lived at the Enola address articulated above, for all of his life.
4. The child himself and your Petitioners all desire that, by order of this Court, the
child's name be changed to: Coby Yong An Huang.
5. The family desires the name change described above for two reasons: first, Yong

is a name within the family's heritage, and Young, as appears on the child's birth
certificate, represents a spelling error; second, most importantly, the child desires, and his
parents support him in this, that his first name be changed to "Coby" as described above
so that the child has an anglo-Saxon first name and can better assimilate with his school
mates and friends.
6. The child is not married and has not had any children in his lifetime.
7. The child has no criminal record; we anticipate obtaining a criminal record check
from the Pennsylvania State Police, even if that is not literally required.
8. There are no judgments or decrees of like character of record, against the child.
9. Neither your Petitioners nor the child know of anyone who objects or, upon notice
would object, to the name change proposed herein.
WHEREFORE, your Petitioners, on behalf of their minorson, requestthis Honorable
Court to set a hearing to consider the requested change of name as contained herein, to
ultimately enter an Order changing the child's name as requested herei5 and to provide
any other relief that this Court deems approp

I, Kuang Huang, father of Young-Un Coby Huang, am verifying this Petition on
behalf of Young-Un Coby Huang, in that the Young-Un Coby Huang is a minor and is not
legally competent to verify court documents. I verify that the facts set forth in the attached
document are true and correct to the best of my knowledge, information and belief. I so
state subject to the penalties of 18 Pa. C. S. §4904 (relating to unsworn falsification to
authorities).
