hereby certify that on the 27th day of December, 2012, I served L. Marlin Eby, Daniel M.
Eby, Michael J. Eby, Rachel E. Eby, and Matthew A. Eby, all parties in interest in the above-
captioned matter, and entitled to share in the estate of the alleged incapacitated person,
with notice of (1) the Petition filed November 29, 2012, to have David James Eby declared
an incapacitated person, and (2) the hearing scheduled on the said Petition by the Court.

I am enclosing an Amended Certification of Service to be filed in the above-referenced matter. I
filed a prior Certification of Service with the court on January 17, 2013, immediately prior to the
hearing in this matter. However, the previously-filed Certification of Service did not include all of
the parties of interest who were served notice of the hearing. Therefore, the enclosed Amended
Certification of Service is intended to supersede the prior Certification of Service.
