RECEIPT AND RELEASE
KNOW ALL MEN BY THESE PRESENTS, that I, JESSICA R. SHEFFER, have
received from Steven M. Garvin, Executor of the Last Will and Testament of Helen P. Garvin,
Deceased, the sum of FIVE THOUSAND ($5,000.00) DOLLARS (the "Property"), in full
satisfaction of the gift made to me in ITEM SECOND, paragraph (A) of the Will of the above-
named Decedent; and
I do hereby fully release and discharge the Estate of Helen P. Garvin, deceased (the
"Estate"), and the said Steven M. Garvin from any and all claims I may have against the Estate or
against Steven M. Garvin for his administration of the Estate; and
I do hereby accept and approve the distribution of the Property with the same force and
effect as if the Property had been distributed in conformity with a decree of court after
confirmation of an account; and
I direct and authorize the Clerk of the Orphans' Court and/or the Register of Wills to
enter a receipt on the docket in my name, place and stead.

Deceased, the sum of FIVE THOUSAND ($5,000.00) DOLLARS (the "Property"), in full
satisfaction of the gift made to me in ITEM SECOND, paragraph (A) of the Will of the above-
named Decedent; and
I do hereby fully release and discharge the Estate of Helen P. Garvin, deceased (the
"Estate"), and the said Steven M. Garvin from any and all claims I may have against the Estate or
against Steven M. Garvin for his administration of the Estate; and
