Upon demand, I promise to pay to Donald C. Donagher, Jr. $8,000.00 without
defalcation, with interest at the rate of eight (8%) percent per annum, value received.
And further, I do hereby authorize and empower the Prothonotary or any Attorney of any
Court of Record in Pennsylvania or elsewhere to appear for and enter Judgment against
me for the above sum, with or without declaration, with costs of suit, release of errors,
without stay of execution, and with 12 percent added for collection fees; and I hereby
agree not to make any motion or any application whatsoever to any Court for an
inquisition on any real estate that may be levied upon to collect the aforesaid sum, and I
voluntarily condemn same, and authorize the Prothonotary to enter said voluntary
condemnation upon the Writ of Execution. I further agree that any property, real,
personal or mixed may be sold through a Writ of Execution and further hereby waive and
release all relief from any and all apprisements, stay or exemption laws of any State now
in force or which are passed hereafter.

DISCLOSURE STATEMENT
I, Joni E. Donagher, executed, this of September,
2004, a Promissory Note for Eight Thousand ($8,000.00) Dollars
obligating me to repay that amount. ?ix -
A representative of Friedman and King, P.C. has explained to
me that the note contains wording that would permit Donald C.
Donagher, Jr. to enter judgment against me at the court house,
whether or not said note is in default, without notice to me and
without offering me an opportunity to defend against the entry of
judgment nd that the judgment may be collected by any legal
means. `
In executing the note, I knowingly, understandingly, and
voluntarily waive my rights to resist the entry of judgment
against me tithe court house and I consent to the confession of
judgment.

$10,000.00, that the blanks in this disclosure were filled in
when I initialed and signed it, and that I received a copy at the
time of signing.
