4. The work, services, materials and/or equipment described as FIRE PROTECTION COMPONENTS were provided
pursuant to the terms of a written contract (or in the case of a supplier, a delivery or order slip signed by the owner,
contractor or subcontractor], dated 9/17/2012 between the claimant and UNITED STATES SPRINKLERS, L' LC having
an address of 317 WESTTOWN RD SUITE 4 WEST CHESTER, PA 19382.
5. The contract price of the contract under which this lien claim is made is $13,754.69.
6. The total of all payments received by the claimant against the contract price is $0.00
7. The amount of this construction lien is $13,754.69
