PETITION FOR NAME CHANGE
AND NOW comes the Petitioner, Michael Albert Brown, by and through
his attorney, Jane Adams Esquire, and respectfully requests this Honorable
Court to change his name to Michael Albert Barone for the following reasons:
1. Petitioner, Michael A. Brown, currently resides at 458 Brook Circle,
Mechanicsburg, Pennsylvania, 17055.
2. Petitioner has been a resident of Central Pennsylvania for over 20 years.
3. Petitioner requests restoration of the family name Barone that was
changed to Brown by his grandfather in the early 1900's. (Please see Exhibit
A, attached).
4. Petitioner has no outstanding credit debt that could be avoided through
this name change.
5. Petitioner has no outstanding federal or state tax debt that could be
avoided through a name change.
6. Petitioner has no knowledge of any company, person or relationship that
will be adversely effected by restoring his name to Barone from Brown.
WHEREFORE, Petitioner respectfully requests your Honorable Court to grant
the foregoing Petition for Name Change and change his name to Michael Albert
Barone.
