DECREE and ORDER of DISTRIBUTION
AND NOW,this IS J day of 2013, upon consideration of
the annexed petition, and pursuant to 20 Pa. C.S. § 3102, distribution of the estate of
Freda J. Harry, so far as it consists of$1,061.14 on deposit in a resident trust account at
Claremont Nursing& Rehabilitation Center, $4,176.38 on deposit in M&T Bank checking
account number 950362742 and a $555.46 refund of health insurance premium, is
hereby awarded to the Pennsylvania Department of Public Welfare, subject to payment
of administrative fees and costs as hereinafter set forth:
1. «Claremont Nursing& Rehabilitation Center and M&T Bank are hereby
directed to make payment of funds on deposit in the name of Freda J. Harry to the law
firm of Flower Law, LLC for deposit into its IOLTA account.
2. The said law firm is authorized to pay therefrom filing fees to the Register of
Wills and to collect $1000 for attorney fees, and is directed to disburse the balance to
the Pa. Department of Public Welfare.
