Execution and Claim for Exemption Form to a person representing themselves to be Rachel Acevedo-
Wife, who accepted as"Adult Person in Charge"for the within named Defendant, to wit: Ralphael
Acevedo at 1162 Heron Court, Hampden Township, Mechanicsburg, PA 17055, informed person of
