CV Secretary FINAL GUARDIANSHIP REPORT Terry Barley WILLIAM GALLAGHER Director r-' rc, Cam' L.1 ._1 %cl "' --1 Cr March 20 2013 �Report from Guardian of Estate and Person T.=nberland County Aging and Community Services Priscilla Whitman, Aging Care Manager 3 On August 27, 2009 Judge Kevin Hess appointed the Cumberland County Aging and Community Services as the permanent Plenary Guardian of Person and Estate for William Gallagher. PERSON Upon appointment, Bill was transported to the Carlisle Manor Care Home. He had been staying in a trailer at a nearby car dealership prior to this. Mr. Gallagher was diagnosed with diagnoses include dementia, anxiety, osteoporosis, mild degenerative joint disease, depression, gastroenterology reflux disorder. Mr. Gallagher continued to suffer from delusions. On one visit he told staff that he had just gotten his pilot's license the night before. Nursing staff assisted with dressing, grooming, personal care and toileting. He required one assist of supervision with transfers. His diet was changed from mechanical soft to pureed because of difficulty swallowing. One of the main activities he enjoyed was the monthly lunch out with other residents. He used to enjoy sitting in the lobby and greet visitors to the facility. His ambulation declined to the point that he is no longer able to walk In the November's Care Plan it was reported that he was continuing to do well medically. At that time he was happy, outgoing, talkative, and always on the go. On February 27, 2013 Bill had a syncopal episode while the physician was in the building. His heart rate was 150 and he was sent to the ER by doctor's orders. He was admitted to the Carlisle Hospital for rapid A-Fib, dehydration and failure to thrive. He was a DNR and DNI. The agency was notified that he had passed away on March 1, 2013. Agency staff had visited with him the 2 days prior to his death. He was currently receiving $210 in Social Security and $2057.86 in federal government pension The balance in his checking_account_at the-time of his death was$730.17. Pre-funeral arrangements had been made with Hollinger Funeral Home Because his balance is less than $2400, the Estate.Recovery Program of the Department of Welfare will not need to be notified. Attached is a final financial summary of William Gallagher's income and expenses.
