including improvements thereon, owned by the Owners and described as follows:

for unpaid sewer and /or trash services rendered from August 1, 2012 through February
28, 2013. Said services were rendered and charged pursuant to Hampden Township
Code of Ordinances Chapter 18, Part 2 and/or Chapter 20, Part 1, as amended.
Attached hereto is a statement of the kind and character of the services so

the Owners' account with the Claimant, which sum of $513.20 was duly assessed against
the above Owners and premises beginning August 1, 2012 and for which sum, which
includes interest, costs and penalties, together with attorney's fees pursuant to Section
209, Part 2, Chapter 18 of the Township of Hampden Code of Ordinances, a lien is
claimed against the above premises in accordance with the Municipality Authorities Act
of June 19, 2001, P.L. 287, 53 P. S. § 5601, et se q., as supplemented and amended and
the Municipal Claims and Tax Lien Act of May 16, 1923, P.L. 207, 53 P.S. § 7101, et
se q.. as supplemented and amended.
