PETITION FOR THE APPOINTMENT OF A GUARDIAN OF THE ESTATE
OF JOSHUA J. PFAUTZ
AND NOW, comes the Petitioner and proposed guardian, Rebecca Quinn, by and
through their counsel Reager & Adler, P.C. and respectfully represents ~t~. ollowipg .:.:
1. The name and address of the proposed guardian of the estat~:0fJosh ~u~.J. Pfa~tz?
(hereinafter "the child") is Rebecca Quinn, natural mother and adult indNidual re~ing at 725
Florence Circle, Mechanicsburg, PA 17050. ~
2. Joshua J. Pfautz is a minor child with a birthdate of November 28, 19~r~. The
child's birth certificate is attached hereto as Exhibit "A" and is incorporated herein by reference.
3. The child's natural mother is Rebecca Quinn, residing at 725 Florence Circle,
Mechanicsburg, PA 17050.
4. The child resides with his natural mother at 725 Florence Circle, Mechanicsburg, PA
17050.
3. The child is a beneficiary under an IDS Life Insurance Company Annuity purchased
by his late grandfather. Said Annuity is administered by American Express Financial Services

and bears the account number 93002501058 6 004.
4. The child's beneficial interest under said Annuity is approximately $22,540.90.
5. The child is also a beneficiary of an IDS High Yield Tax Exempt Mutual Fund
purchased by his late grandfather. Said Mutual Fund is administered by American Express
Financial Services and bears the account number 01151027691 5 002.
6. The child's beneficial interest under said Mutual Fund is $71,696.00.
7. American Express Financial Services requires that a court order evidencing the
guardianship of the estate be produced prior to funds being disbursed to a minor beneficiary.
8. Harry V. Pfautz, during his lifetime, established a custodial account for Joshua
Pfautz. Said custodial account was established at Merrill Lynch and bears the account number
898-10A42. Said account has a value of approximately $17,443.10.
9. The proposed guardian requests that she be court appointed as the Guardian of the
Estate of her son, Joshua J. Pfautz, so that his beneficial interests as referenced above, may be
released and held for his benefit.
10. The proposed guardian further requests that the Court direct Merrill Lynch to
replace Rebecca Quinn as the custodian on the Merrill Lynch account number 898-10A42 so that
she may hold it for his benefit.
11. The proposed guardian has knowledge that the child does possess a financial estate
of approximately $196,946.65. An inventory of said assets is attached hereto as Exhibit "B"
12. The proposed guardian has no interest adverse to the child.
