AND NOW, comes Sarah Asaad, Natural Mother of James Kentland Affleck, by
and through her Attorney, Jane Adams, Esquire, and respectfully requests this
Honorable Court to change his child's name to James Kentland Boling for the following
reasons:
1. Petitioner , Sarah Asaad, (hereinafter"Mother"), is the Natural Mother of
James Kentland Affleck, (hereinafter"Child"), born March 21, 2010 in Winchester,
Virginia. She lives with the child at 25 Partridge Circle, Carlisle, Cumberland County,
Pennsylvania, 17013.
2. The Natural Father of James Kentland Affleck (hereinafter"Father"), is Varun
Tandon, who is active duty in the U.S. Navy, currently stationed out of San Diego,
California. Paternity was confirmed by genetic testing.
3. The child was born while Mother was going through the process of divorcing
her ex-husband, Ali M. Asaad.
4. When the child was born, Mother was engaged to marry Matthew Affleck,
hence Mother named the child James Kentland Affleck, anticipating that she and her
son would have the same last name.
5. Mother ended the relationship with Matthew Affleck in June of 2012.
6. The child now has a last name which is different from his Natural Father and
Natural Mother.

7. Mother believes that Father will be in agreement with the name change and
she will serve a copy of this petition and the hearing notice upon him via certified mail.
8. Since the child is only three years old, there is no outstanding credit or tax
debt that could be avoided through a name change.
9. Mother is requesting that the requirement to advertise notice of the name
change petition be waived due to the child's age.
10. Mother is requesting that the child's last name be changed to her maiden
name, Boling.
11. Mother, Sarah Asaad, is requesting that this Honorable Court change her
child's name to: James Kentland Boling.
WHEREFORE, Petitioner, Sarah Asaad, requests that this Honorable Court
Order change the child's name.
