Petition for Grant of Letters of Administration pendente lite
On April 2, 2011,the decedent was operating his motor vehicle at approximately 0120 hours,traveling south in
the northbound lanes of Interstate 81, with a blood alcohol content of.28. He died at the scene as a result of
colliding with various northbound vehicles. Petitioner requests the issuance of temporary Letters of
Administration pendente lite for a period of sixty(60)days for the sole purpose of facilitating the commencement
of a personal injury lawsuit against the personal representative of the decedent on behalf of one of the victims of
the decedent,Ginger A. Clark,with the understanding that the plaintiff in the intended lawsuit will,within sixty
(60)days of the issuance of the Letters of Administration pendente lite,take the necessary steps to have petitioner
removed from the position of personal representative and substituted by another qualified party. Petitioner does
not have a copy of the death certificate of the decedent. Petitioner proposes to prove the death of the decedent
with the attached copy of the Commonwealth of Pennsylvania Police Crash Report at Incident# H03-2017409.
Except as follows: Decedent was not a party to a pending divorce proceeding wherein the grounds for divorce had been established as
defined in 23 Pa.C.S. §3323(g)and was neither the victim of a killing nor ever adjudicated an incapacitated person.

NARRATIVE
This collision occurred as Unit#i was traveling south in the northbound lanes of IntersWe 81 and Unit#2 was
traveling north In the left northbound lane when UnIt;#1 struck Unit#2 head on. Unit*M then traveled Into the right
northbound lane and was struck In the left side by the front of Unit#3. Unit 01 then came to final rest in the left
northbound lane of travel facing In a south easterly direction. Unit#2 came to final rest upon impact in the left
northbound lane of travel facing in a north easterly direction. Unit#3 pulled over to the tight berm of Interstate 81
northbound facing In a northerly direction.

traveling north in the left northbound lane when Unit#1 struck Unit#2 head on. Unit#1 then traveled into the right
