AND NOW, comes John A. Abom, Esquire, of Abom& Kutulakis, LLP, on behalf of the Petitioner, Sharon Lantz, in re Seth Thomas Clouser, who respectfully requests this Court allow Seth Thomas Clouser to change his surname to Seth Thomas Lantz, and in support thereof avers as follows: 1. Petitioner is Sharon Lantz, an adult individual who resides at 575 Ridge Road, Lewisberry, York County, Pennsylvania. 2. Petitioner seeks the within Name Change Petition for her son, Seth Thomas Clouser, who currently resides with his mother, Petitioner Lantz. 3. Petitioner is the guardian and natural mother of Seth Thomas Clouser, born May 11, 2001. Petitioner desires to change her son's surname from Clouser, to her last name, Lantz. 5. The child was born out of wedlock and the Petitioner is not married to the child's natural father. 6. The Petitioner has been the primary caregiver of the child since birth. 7. The Father of the child, Timothy F. Clouser, does not contribute to the child's financial support. at the following addresses: Name Sharon Lantz Timothy Clouser During the child's lifetime, he has resided with the following persons and Address 100 W. On August 31, 2004 This Court adopted a Stipulated Order for Custody granting full legal and physical custody to Petitioner Lantz. 10. The Father, Timothy F. Clouser, does not oppose the within Petition. 11. Changing the child's name from Seth Thomas Clouser to Seth Thomas Lantz is in the best interest of the child. 12. As the child is not yet of school age and not mature enough to express his preference, the child's name should be changed now to eliminate confusion and in the best interest of the child. 13. Petitioner requests this Court waive the requirement for official searches of the proper offices of the county wherein the Petitioner and child reside, as required by 54 Pa.C.S. § 702(b), on the grounds that the subject of this petition is a minor and avers that there are no judgments or decrees of record or any other matter of like character against him. 14. A copy of this Petition has been served by mail upon Timothy Clouser and Petitioner intends to serve him with all notices of any scheduled heating pursuant to this Petition.
