PETITION PURSUANT TO 20 PA C.S. §5111 FOR APPOINTMENT OF
GUARDIAN OF THE ESTATE OF KATHERINE ROSE DUFFIELD,MINOR.
To The Honorable, the Judges Of Said Court:
Katherine Rose Duffield petitions this Honorable Court for an order appointing her aunt,
Sara A. Mascho, guardian of the Estate of Katherine Rose Duffield, Minor, and in support
thereof, avers as follows:
1. Katherine Rose Duffield("Katherine") is a minor, age 15,born on January 9, 1998,
who lives with her parents, Robert S. Duffield and Joan M. Duffield at 207 Edenderry Way,
Enola, PA 17025. Her residency in Cumberland County confers jurisdiction upon this Court
pursuant to 20 Pa. C.S. §5111(a).
2. Sara A. Mascho, age 47,the proposed guardian of the estate, is Petitioner's aunt, and
resides at 463 Stacey Drive, King of Prussia,PA 19406.
3. The appointment of a guardian of the estate for Katherine is necessary to receive the
proceeds of an annuity policy from Transamerica Life Insurance Company, payable to Katherine
on the death of her grandmother, Delores Elizabeth Bergin, Deceased.
4. No guardian of the estate has been previously appointed for Katherine.
5. Sara A. Mascho, the proposed guardian of the estate for Katherine, has no interest
adverse to Katherine.
6. Robert S. Duffield and Joan M. Duffield consent to the petition filed by Katherine
requesting the appointment of her aunt, Sara A. Mascho as guardian.

7. Katherine's Estate will be comprised of the following asset: Transamerica life
insurance proceeds of approximately Thirty-Five Thousand($35,000) Dollars. Interest is
accruing on this account, and the exact amount will not be determined until the date of
distribution.
7. Attached hereto as Exhibit"A" is the written consent of Sara A. Mascho to serve as
guardian of the estate for Katherine.
WHEREFORE, Petitioner respectfully requests this Honorable Court to appoint Sara A.
Mascho guardian of the estate for Katherine Rose Duffield.
