AND NOW, this 4t' day of April, 2013, upon consideration of the attached
Petition for Change of Name, a hearing is scheduled for Thursday, June 13, 2013, at 9:30
a.m., in Courtroom No. 5, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioners shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, §6 (see 54 Pa. C.S. §701 [Historical and

ORDER OF COURT
AND NOW, this 13th day of June, 2013, this being
the time and place set for a hearing on the Petition for
Change of Name, and this Court having determined that the
requirements of Title 54, Section 701 have been satisfied,
and the Court further being in receipt of a consent filed by
the father of the minor child that the father consents to
this name change, it is hereby ordered and decreed that the
name of the minor child, which is currently Lucas Aaron
Wilson, is hereby changed to Lucas Aaron King-Wilson and
that the minor child shall from this date forward
exclusively use and be known by the name of Lucas Aaron
King-Wilson.
