I, Connie Stoneroad, President of Keystone Guardianship Services, do hereby verify that
I personally served a true and correct copy of the Petition for Appointment of Permanent
Plenary Guardian of Person and Estate and Important Notice Citation with Notice, dated
February 20, 2013 upon the alleged incapacitated person, Pauline K. Cover, by hand delivery
on the I Q day of 2013 at/�-'y ` at the following address: Golden
Living Center-West Shore, 770 Poplar Church Road, Camp Hill, Pennsylvania, 17011 by
making known the contents thereof to the alleged incapacitated person and handing to and
leaving with the alleged incapacitated person a copy of the Preliminary Order.
This Affidavit is made subject to the penalties of 18 Pa. C.S. § 4904 relating to unsworn
falsification to authorities.
