DECREE
AND NOW,this day of �� , 2013,upon consideration of the Petition
heretofore filed by R. Jane Cassidy, it is hereby Ordered and Decreed:
1. That R. Jane Cassidy is authorized to close the checking account#51-4039-
8455 at PNC Bank in the amount of$3,102.52 plus any accrued interest thereon;
2. That R. Jane Cassidy is authorized to close the savings account#51-3031-9725
at PNC Bank in the amount of$7,213.37 plus any accrued interest thereon;
3. That PNC Bank is Ordered to issue a check made payable to R. Jane Cassidy
for the proceeds of both accounts referred to hereinabove and R. Jane Cassidy is
authorized to close, endorse and execute any and all documents required by PNC Bank to
close said accounts; and
4. That R. Jane Cassidy is authorized to execute any and all tax documents that
may be needed to be signed in order to comply with the laws of the Commonwealth of
Pennsylvania,the United States of America, and any local government.
Payment and distribution are accordingly Decreed and R. Jane Cassidy,the Petitioner, is
hereby authorized to receive,collect and distribute the property as herein before set forth.
