North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
1
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The name of the owner of the property against which this lien is filed is Alan G. Davis, II of 205
Dunbar Drive, Mechanicsburg, Pennsylvania 17050 -2479.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 514 Longs Gap Road, Carlisle, Pennsylvania.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid water and sewer user fees against the above - described property in the
amount of $428.22, plus any additional fees incurred thereafter.

Interest is claimed on the above - mentioned fees until paid in full.

along Lot 3, North 23° 53 minutes 44 seconds East, 88.22 feet to an lot pin, set, at corner of
same; thence along same, South 66° 39 minutes 15 seconds East, 188.90 feet to a corner of said
Lot 3 and Longs Gap Road; thence along Longs Gap Road, South 23° 47 minutes 30 seconds
West, 71.63 feet to a point; thence along said Longs Gap Road, on a curve to the left having a
radius of 335.66 feet, a chord bearing of South 8° 7 minutes 5 seconds West, and an arc length of
181.91 feet, to a point, the place of BEGINNING.
BEING the same premises which Dirk L. Zechman, by deed dated October 15, 2008 and
recorded in the Office of the Recorder of Deeds in and for Cumberland County at Instrument
200834874, granted and conveyed to HD Enterprises, LLC, the Grantor herein.
AND the said Grantor does hereby covenant and agree that it will warrant SPECIALLY
the property hereby conveyed.
