Cons. Stat. Section 701, et seq., and in support thereof makes the following the averments: c C - 1. The petitioner was born on 04/19/1995 and is over the age of eighteen years. �. rn rn 4"1 rn 2. During the five years preceding the tiling of this petition, the petitioner has resi &p2 in the following place: Current - 124 Old Schoolhouse Lane, Mechanicsburg, PA 17055 _ 3. There are no judgments or decrees pending against the petitioner. _ ; 4. The Petitioner requests that his name be changed from Nam Wook Cho to Martin Namwook Cho 5. The reasons for the requested change of name are: His first name was `Namwook' but now Nam is first and Wook is middle name by mistake of his parent. It is incorrect and he really wanted to have new American style name in school life. He chooses `Martin' as first name and `Namwook' as middle. All his friend and teachers can easily call him and he will love to hear new name `Martin'.
