Stat. Section 701 et seq., respectfully sets forth that: 1. Your Petitioner, Martin Joseph Mangan, IV (hereinafter referred to as "Petitioner"), is 21 years old, and was born March 9, 1983, in Forbes Regional Medical Center in Monroeville, Pennsylvania. 2. Petitioner is a current resident of Mechanics burg, Cumberland County, Pennsylvania, and has been for the past five years during regular academic school years as he is a student at Dickinson College. Petitioner is also a resident ofMurrysville, Westmoreland County, Pennsylvania for time periods when school is not in session. For the five years preceding the filing of this Petition, Petitioner has resided at the following locations: Date Address 2001 to present (academic school year) 1428 Apple Drive, Apt. #199 Mechanicsburg, PA 17055 2001 - 2003 (summers) 5069 Northlawn Circle Murrysville, PA 15668 1983 - 2001 (year round) 5069 Northlawn Circle Murrysville, P A 15668 3. Petitioner resides at 1428 Apple Drive, Apartment #199, Mechanicsburg, Cumberland County, Pennsylvania 17055. 4. Petitioner's birth parents are Beverly Rae Leopold, and Martin Joseph Mangan, III. 5. Petitioner's parents divorced January, 1989. 6. Since that time, Petitioner's mother retook her maiden name of Leopold. 7. Petitioner has not had any substantial contact with his biological father for at least seven years. 8. Petitioner is now an adult, over 18 years of age, and desires to separate himself from his biological father. 9. The name Petitioner desires to take is his mother's maiden name. Petitioner desires to have the same name as his mother now has. 10. Because Petitioner desires that his surname reflect that of his mother, i.e. "Leopold", and it is Petitioner's desire to separate himself from his biological father, Petitioner desires to be known as Julius Joseph Leopold. 11. There are no judgments or decrees of like character against Petitioner.
