Feeney Revocabie Trust Agreement,otiginatly dated September !2, 199'7, as amended and
restated on March 20, 1998, and as subsequently amended os� September l2, 20Q0 and on
September 14, 2001 (collectively, the ""I"rust AgreemenC"), established by Thomas J. Feeney and
Mildred L. Feeney as Co-Settlors, you are hereby notified of the foilowing particulars:
1. The Trust was created by Thomas J. Feeney and Mildred L. Feeney, as Co-
Settlars, by that certain Revocabie Tnsst Agreement,originally dated September 12, 1997, as
amended and resiated in its entirety on March 2Q 1998, and as subsequently amended on
September 12, 2000 and on SepYember 14, 2001, with 7'homas J. Feeney as original Trustee.
2. Original Truscee and Co-Settlor Thomas J. Feeney died an Apri129, 2003, a
resident of Curnberland County, Pennsylvania,
�. By that certain Acceptance and Appointment dated June 7, 2003, George A.
Seamands became sole Successor Trustee of the Trust.
4. Aceording ta its terms, the Tnzst Agreement becarne inev�eable in 2063, upon the
death of Co-Settlor Thomas J. Feeney.
5. Co-Sattlor Mildred L. Feeney d'aed March 31, 24]3. The Register of Wills of
Cumberland County, Vennsylvania, has issued Letters Testamentary to Robert R. Church and
George A. Seamands, as Co-Executors of the Estate of Miidred L. Feeney, deceased.
6. In accardance with law, the follawing particulars are provided concerning the
Trustee's cantaet infprmation: George A. Seamands, Successor Trustee, 9214 Sztter Piace,
Springfield, VA 22 1 53-1 025.
7. As a beneficiary, you have the right ta receive a copy of the Trust Agreement,as
well as the right to xeceive, at least annually, a written report of the assets of the Trust, indicating
their rnazket value (if feasible), as well as the Trust's lit�bilities, receipts and disbursements since
the date of Yhe last such report by the Trustee. Yoa aiso have the right ta waive your right to
reeeive sueh noflcas,
