As the administrator of the above estate, I am sending you (1)two copies of PA form REV-1500,
Inheritance Tax Return—Resident Decedent and attachments thereto and (2)two copies of the
Inventory, Form RW-09.
As indicated in box 9 of the REV-1500 Form, the entire assets of this estate relate to the
proceeds received from litigation in March of this year. Because the amount of the litigation
proceeds were unknown until the date of receipt, REV-1500 could not be filed within the
statutory due date of 9 months from date of death. As the administrator of the estate,I am
requesting that the Register of Wills consider applying an early payment discount of 5%of the
inheritance tax because the tax is being paid within the three-month period from which the
litigation proceeds were received. if the Register of Wills is agreeable to applying the early
payment discount, please consider this as a request for refund of the tax that was overpaid.
Included with the filing of Form REV-1500, are the following documents:
• Summary of Litigation Proceeds and Disbursement of Monies made by the estate
• Statement Regarding Interest Computation
• Copy of Short Certificate issued by the Register of Wills showing me as the administrator
of the estate
• Orphan's Court order authorizing settlement of underinsured motorist claim and
attachments thereto
• Settlement statement relating to the Common Pleas Court of Dauphin Count and
attachments thereto
• Various medical bills
• Funeral bill and proof of payment
