1. MARCIA COLETTE AGRAFIOTIS (also known as Marcia Agrafiotis) was
married to ANGELO AGRAFIOTIS.
2. The said Marcia Agrafiotis was divorced from Angelo Agrafiotis on April 26,
1978, in the Superior Court for the Judicial District of Fairfield in the State of Connecticut, a
duly certified copy of the judgment dissolving said marriage being heretofore filed in this matter.
3. The said Marcia Agrafiotis is an adult individual and is a resident of the Township
of Hampden, Cumberland County, Pennsylvania, having an address of 1339 Concord Road,
Mechanicsburg, Pennsylvania 17050.
4. The said Marcia Agrafiotis desires to resume use of her maiden name, to wit:
