PETITION FOR ISSUANCE OF MARRIAGE LICENSE
AND NOW this- Vj day of May, 2013, come Petitioners, Shellie Thompson and
Eric Maxwell, by and through counsel, Lawrence J. Rosen, Esquire, and offers the following
averments in support of the withing Petition for Issuance of Marriage License:
1. Petitioner, Shellie Thompson is an adult female residing in Dauphin County,
Pennsylvania at 630 Morningside Way, Harrisburg, PA 17113
2. Petitioner, Shellie Thompson, is currently a widow.
3. Petitioner, Eric Maxwell, is and adult male. Petitioner, Eric Maxwell has never been
married.
4. Petitioner, Eric Maxwell, currently resided in the State Correctional Institution at
Camp Hill, Cumberland County, Pennsylvania.
5. Petitioners are desirous of entering into marriage.
6. SCI Camp Hill administration has been notified of Petitioner, Eric Maxwell's, desire
to marry Petitioner, Shellie Thompson, and has approved. (See Exhibit A)
7. In order to proceed, it will be necessary for Your Honorable Court to issue an Order
authorizing a representative from the Register of Wills office to visit Petitioner,Eric
Maxwell, at SCI Camp Hill to personally verify his signature on the marriage license
application.
8. As per the requirements found in the guidelines disseminated by the Register of Wills,
Petitioner, Eric Maxwell,has appropriate identification.
9. Petitioner, Eric Maxwell, understands that he will be required to place the
representative from the Register of Will's office who will be visiting the prison to
obtain his signature on the marriage license on his visitors' list.
WHEREFORE, Petitioners Eric Maxwell.and Shellie Thompson ask that Your Honorable
Court grant their Petition and issue an Order authorizing a representative from the Register of
Wills to visit Petitioner, Eric Maxwell, at the State Correctional Institution at Camp Hill to
obtain his signature on a marriage application.
