AND NOW, this 31st day of May, 2013, up..p-t ,
consideration of the Petition for Adjudication of Incapacity and
for the Appointment of a Plenary Guardian of the Person and Estate,
and after a hearing on this matter on today' s date, Nicole L. Zomok
is hereby adjudicated an incapacitated person. The Court hereby
finds that clear and convincing evidence was presented that Nicole
L. Zomok should be adjudicated an incapacitated person. The Court
further finds that evidence was presented that Nicole L. Zomok
suffers from a condition called 22q deletion, that is a genetic
syndrome that requires her to have constant care, and that this
syndrome will be for the remainder of her life and will not be
curable . Accordingly, Raymond P. Zomok and Christine G. Zomok are
appointed permanent plenary guardians of both the person and estate
of Nicole L. Zomok. The guardians are directed to file reports in
accordance with the provisions of Title 20, Section 5521 (c) on each
occasion that a report is due by statute.
Notice of this matter shall be conveyed as best as
possible to Nicole L. Zomok by the guardians . The guardians shall
also give her her notice of right to appeal and to petition to
modify or terminate the guardianship created herein.
