NO. ,~. i - ©~- i L,.,~ PETITION FOR APPOINTMENT OF LEGAL GUARDIANSHIP AND NOW come the Petitioners, William C. Knoche, and Debra S. Knoche, husband and wife, (hereinafter "Petitioners"), by and through their attorney, Valerie J. Faden, Esquire, and aver the following in support of the Petition to be appointed legal guardians of ERIC CARLOS ARRIAGA, (hereinafter ~Edc"): 1. Petitioner William C. Knoche is an adult individual who currently resides at 218 Birch Lane, Carlisle, Pennsylvania, 17013. 2. Petitioner Debra S. Knoche is an adult individual who currently resides at 218 Birch Lane, Carlisle, Pennsylvania, 17013. 3. Eric was bom on December 31, 1994. A copy of his Birth Certificate is to obtain health care coverage for Edc if they have legal guardianship. 8. Petitioners will also be able to make other major decisions about Edc's health and education if they are appointed Eric's legal guardians. 9. Edc's mother has consented to Petitioners' request to become Eric's legal guardians. The Consent of Ericka Morales to the appointment of Petitioners to legal guardianship is attached hereto as "Exhibit A." 10. Eric's father has failed or refused to execute a Consent to legal guardianship. Edc's father, Cados Arriaga, Jr., is currently serving a prison term of 25 11. Petitioners do not have a fiduciary relation to Edc Carlos Ardaga; nor do they have any interests that would be adverse to Eric Carlos Ardaga. WHEREFORE, Petitioners requests this Honorable Court to issue an Order appointing William C. Knoche and Debra S. Knoche legal guardians of Eric Cados Ardaga. We vedfy that the statements made in the foregoing Petition For Appointment of Legal Guardianship are true and correct to the best of our knowledge, information and belief.
