Plaintiff, AMERICAN HOME ASSURANCE COMPANY (hereinafter "plaintiff') is authorized to do business under the laws of the Commonwealth of Pennsylvania having a mailing address of P. O. Box 382014, Pittsburgh, PA 15250. 2. Defendant, MORRISON & SONS, INC. TRANSPORTATION SERVICES a/kla MORRISON & SONS, INC. (hereinafter "defendant") is a corporation operating under the laws of the Commonwealth of Pennsylvania with offices at 3242 Enola Road, Carlisle, Cumberland County, Pennsylvania 17013. 3. Before on or about October 2,2002, defendant made application to plaintiff for a workers compensation and employer's liability policy. 4. In response to defendant's application. plaintiff issued policy numbers WC990610 and WC00292494100 covering the policy period of October 2,2002 to October 2.2003. 5. Premiums for each policy are based upon an end term audit which is conducted by plaintiff utilizing the books and records of the defendant. 6. After all credits were applied, a balance of $12,579.00 remains due plaintiff from defendant for the earned premiums as well as annual and additional premiums. 7. Attached hereto, made a part hereof and marked as Exhibit "1" is a true and correct copy of the policy information page, audit advice summary and invoice, which reflect the total balance remaining due of $12,579.00 which defendant has failed or refused to pay despite repeated demands for same. 8. In addition to outstanding principal, plaintiff demands interest at the legal rate of 6% from an average due date of February 6, 2004. WHEREFORE, plaintiff requests judgment in its favor and against the defendant in the amount of $12,579.00 plus interest at a rate of 6% per annum thereon from an average due date of February 6, 2004 plus costs.
