PETITION FOR SETTLEMENT OF SMALL ESTATE
To the Honorable Judge of the Orphans' Court of Cumberland County,
Pennsylvania:
The Petition of Lori Lee Jakubs,Administrator of the estate of Carolyn J. Arrigo, by and through
her counsel, Mark A. Mateya, Esquire, respectfully represents:
1. Petitioner is Lori Lee Jakubs,who is the daughter of Carolyn J. Arrigo,deceased, and has
the following interest in the assets that would comprise the estate of Carolyn J. Arrigo:
a• Bank accounts with Wells Fargo which have a combined balance of$24,957.66.
2. Carolyn J. Arrigo, died intesta.te on February 16, 2013, at Carlisle, Cumberland County,
Pennsylvania, and at the time of death the decedent's last principal residence was 238 West North Street,
Carlisle, Cumberland County, Pennsylvania.
3. The above-named decedent died intestate and no letters of administration have been �
issued.
4. The names, addresses, and relationships of all persons having an interest in the estate of
the decedent as heirs or next of kin are as follows:
a. Lori Lee Jakubus—818 14th Street, Apt I, Virginia Beach, VA 23451 —Daughter;
b. Phillip Arrigo—21640 Liberty Street,Apt 1808 A, Lexington Park,MD 20653 —Son.
5. The decedent was not survived by any persons entitled to clai��the famil�xe}a��n

6. The total va.lue of the decedent's personal esta.te is less than$25,000.00 and consists of
the following assets which have the following values:
a. Wells Fargo—Checking Account No. 1010059557140 with an account balance of
$23,902.54;
: b. Wells Fargo—Savings Account No. 3000216716826 with an account balance of
$1,055.03.
7. The following is a list of all known, unpaid creditors and the amount of their claims,
which claims are proposed to be paid from the assets of the decedent:
a. Hoffman Roth Funeral Home—with an amount due of$2,629.3 8;
b. Jeffrey S. Austin—cleaning/damages at last residence $1,732.61;
c. Mateya Law Firm- Legal Fees for estate administration- $1,200.00
d. PA Dept. of Revenue—Estimated Inheritance Tax- $998.30;
e. UGI Gas Service—Electric service at residence in the amount of$145.93.
f. Stardust Storage—Storage rental for decedent's personal/household effects - $120.00.
g. Lori Lee Jakubs—Expenses associated with administration of decedent's estate -
$2,830.89.
8. The Pennsylvania lnheritance T�Return has not been filed and the inheritance tax due
upon the decedent's estate has not been paid.
9. It is proposed that the following distribution of the decedent's esta.te be made to the
following creditors,heirs or next of kin:
a. Hoffman Roth Funeral Home—with an amount due of$2,629.3 8;
b. Jeffrey S. Austin—cleaning/damages at last residence $1,732.61;
c. Mateya Law Firm- Legal Fees for estate administration- $1,200.00

d. PA Dept. of Revenue—Estimated Inheritance Tax- $998.30;
e. UGI Gas Service—Electric service in the amount of$145.93;
f Lori Lee Jakubs - $2,830.89 (expense reimbursement) and one-half of the residual estate;
g. Phillip Arrigo—One-half of the residual estate.
WHEREFORE,Petitioner respectfully requests this Honorable Court to decree the distribution
of the decedent's personal esta.te to the persons entitled thereto as set forth in Paragraph 9 above.
