O. Box 8300, Fort Washington, PA 19034 ( "Owner ") and all that certain tract of land including improvements .thereon, owned by the Owner and described as follows: 4702 N. Clearview Drive Camp Hill, PA 17011 Tax Property Map: 10 -21 -0279 Parcel No.: 113 for unpaid sewer and/or trash services rendered from August 1, 2012 through March 31, 2013. Said services were rendered and charged pursuant to Hampden Township Code of Ordinances Chapter 18, Part 2 and /or Chapter 20, Part 1, as amended. LAW OFFICES Attached hereto is a statement of the kind and character of the services so SNELBAKER & BRENNEMAN,P.C. 3 7/7 rendered and the prices charged therefor, said statement being a true and correct copy of the Owner's account with the Claimant, which sum of $607.28 was duly assessed against the above Owner and premises beginning August 1, 2012 and for which sum, which includes interest, costs and penalties, together with attorney's fees pursuant to Section 209, Part 2, Chapter 18 of the Township of Hampden Code of Ordinances, a lien is claimed against the above premises in accordance with the Municipality Authorities Act of June 19, 2001, P.L. 287, 53 P. S. § 5601, et sect as supplemented and amended and the Municipal Claims and Tax Lien Act of May 16, 1923, P.L. 207, 53 P.S. § 7101, et seq., as supplemented and amended.
