Prior to this appointment Mrs. Ceja resided in her own apartment at 1108 Yverdon Drive Apt. C4 Camp Hill, Pennsylvania. She was admitted to the Forest Park Health Center Carlisle on May 27, 2005 after the outcome of an Emergency Guardianship hearing. Mrs. Ceja continues to reside at Forest Park Health Center. She was moved from the dementia unit to the nursing unit in August 2007. Due to her severe cognitive loss she was no longer benefiting from the activities and structure of the dementia unit. She continues to be diagnosed with senile dementia Alzheimer's type, depression, Parkinson's, PVD, osteoporosis, hypertension and anxiety. Her medical and cognitive status are stable. She continues to be nonverbal as well as : _ non-ambulatory. The staff at Forest Park Health Center provides 24 hour supervision, maximum hands on assistance with activities of daily living. Theresa is a 2 person assist with the Hoyer lift for all transfers. Because she is a high risk for skin breakdown she has a pressure relief cushion on her wheel chair. In her bed she has a foot cradle to alleviate toe and heel pressure and is repositioned every finro hours. She receives Range of Motion exercises twice a day. She is currently wearing bilateral upper extremity elbow splints to prevent contractures. She sees a dentist and a podiatrist on an as needed basis and her doctor on a basis. Mrs. Ceja no longer sees the psychiatrist as she has been emotionall st blelne has not displayed behavioral problems. She is taken to activities, includin Mass b and not responding in any way to them. 9 , ut �s Mrs. Ceja is no longer on a regular diet but is on a pureed diet with thin liquids. She eats about 75-100% of her meals and requires total assistance at mealtime. She receives fortified milk and an ice cream supplement. Client is visited every 30-60 days by the care management staff. in addition to visiting client, care plans are attended every 90 days and the medicaf record is reviewed and discussed with the nursing staff. The staff at the nursing home provides updates and notifications of any changes that Theresa may have experience. . It is the request of the Cumberland County Office of Aging that the Agency retains Guardianship of Person and Estate of Theresa Ceja. Forest Park Health Center has demonstrated an adequate quality of care for client by managing her daily needs and continuously monitoring her medica! conditions. Mrs. Ceja's appearance is always good and she appears to be content in her present living situation. Therefore, we believe that Mrs. Ceja shoufd continue to reside at Forest Park Health Center. ESTATE Funeral arrangements were made with Hollinger Funeral Home in Mt. Holly. Mrs. Ceja has an irrevocable burial trust with Sovereign bank. Payments were made to Forest Park health Center for her room and board as directed by Medical Assistance. Payment is currently $708.00 a month. The Office of Aging also received $100 a month for Guardianship fee as provided for in the guardianship court order. Ghristmas gifts were donated by Home Instead, a home health agency. Her currently income includes $225.44 from a state annuity, $158 state civil service and $708 from Social Security.
