Cumberla�! County Aging and Community Services was named as the Permanent Plenary Guardian of Person and Estate for Sibyl Arms. PERSON Following the hearing Ms. Arms was returned to the Golden Living-West Shore Center where she had been residing since being removed from her home by Emergency Involuntary Intervention. Her house had been condemned by the Borough of Camp Hill as unfit for human habitation. She underwent surgery at Hershey Medical Center to remove an old shoulder implant because it had become infected. She ended up having a full shoulder replacement. From Hershey she was transferred to Holy Spirit for gall bladder surgery. From there she was admitted to Claremont Nursing and Rehab Center on January 21, 2011 where she continues to reside. She has been diagnosed with senile dementia Alzheimer's type, mild hypertension, generalized weakness, anxiety, depression/bipolar disorder, right hand contractures, functional incontinence dysphagia, and dementia In May she was placed on an anti-depressant that seemed to help her mood for a while. She became more social and seemed to enjoy getting out of her room. She likes to make sure her jewelry matches her outfits. At that time she was feeding herself about 50% of the time and enjoyed activities such as Bingo, singing, ball and balloon games, Bible study and outdoor trips. For the "Christmas in August" celebration, a flat screen TV, shelving unit and a bedspread were purchased for her. By September she started to experience a decline and was placed on a pureed diet. Her occupational therapy was increased to 3 times a week for self-participation with positioning and care. She continued to decline and in October it was felt that it would be in her best interest to be placed on Hospice. Her decline was not due to any medical condition but is believed to be due to her dementia. The nurse from Grane Hospice sees her twice and week and an aide assists 3 days a week. The family was notified of her decline and her sister from Georgia and a niece from the Chicago area came to visit in October. She continues in this state. She opens her eyes and looks at Aging staff when they visit and will occasionally nod her head. She does not appear to be in any discomfort. In April she got a new, male aide from Grane Hospice whom she seems to like and responds more to than the other aide. He gets her up and out of her room more. It is the request of the Cumberland County Aging and Community Services that the agency retain Guardianship of Person for Sibyl Arms. The Claremont Nursing and Rehabilitation center has demonstrated an adequate quality of care for her medical problems. We believe, therefore that Ms. Arms should continue to reside in that nursing facility. ESTATE A guardianship account was opened at Wachovia Bank where Ms. Arms had her accounts. In October 2010, Ms. Arms' house and property were sold for$47,800.00, which was $5,800.00 over the appraised value. Application for Medical Assistance was made at the time of her admission to Golden Living. It was discontinued at the time of the sale of the house until her assets dropped below $8000.00. Prior to that, we were required to cash in her policy with Bankers Life and Casualty which has a cash surrender value of$237.50. Re-application was made . for Medical Assistance which began in May 2011. Sibyl's income includes Social Security which was $1017 during 2012 and increased to $1031 in 2013. She also receives a retirement annuity which is currently$ 533.85 a month. Her monthly payment to Claremont Nursing and Rehab for room and board is $1420.17. In addition to the cost of her room and board at the nursing home, a guardianship fee of $100 a month is paid to Cumberland County Aging and Community Services. Sibyl was an avid animal lover, especially cats. She had several. In April, a donation was made to the Humane Society of Harrisburg in her honor. As stated earlier, a TV, shelving and some personal needs items were purchased for her in October. The ' balance in her account at the end of March 2013 was $5929.49.
