To the Honorable Judge of the Orphans' Court af Cumberland Caunty,
Pennsylvania:
The Petition of Lori Lee Jakubs,Administrator of the estate of Carolyn J. Arrigo,by and through
her counsel, Mark A. Mateya,Esqwixe,respectfully represents:
1. Petitioner is Lori Lee Jakubs,who is the daughter of Caralyn J. Arrigo, deceased, and has
the following interest in the assets that would carnprise the estate af Carolyn J. Arrigo:
a. Bank accounts with Wells Fargo which have a combined bala.nce of$24,957.66.
2. Carolyn J. Arrigo, died intestate on February 16,2013, at Carlisle, Cumberland County,
Pennsylvania, and at the time of death the decedent's last principal residence was 238 West North Street,
Carlisle, Cumberland Caunty,Pennsylvania.
3. The above-narned decedent died intestate and na letters of ac'�ministration have been
issued.
4. The names,addresses, and relationships af all persons having an interest in the estate af
the decedent as heirs or next of kin are as follows:
a. Lori Lee 3akubus—$18 14th Street,Apt I,Virginia Beach,VA 23451 —Daughter;
b. Phillip Arrigo—2164Q Liberty Street,Apt 1$08 A, Lexingtan Fark,MD 2Q653—Son.
5. The decedent was not survived by any persons entitled to claim the family exemption
under 20 Pa. C.S. A. Section 3121.
6. The total value af the decedent's personal estate is less than $25,000.00 and consists of
the follawing assets which have the follawing values:
a. Wells Fargo—Checking Account No. 1010059557140 with an account balance of
$23,902.54;
b. Wells Fargo—Savings Account No. 3000216716$26 with an accaunt balance af
$1,O55.Q3.
7. The following is a list of all knawn, unpaid creditors and the amount of their claims,
which claims are proposed to be paid frorn the assets af the decedent:
a. Hoffrnan Roth Funeral Horne—with an amount due of$2,629.38;
b. Jeffrey S. Austin—cleaning/damages at last residence$1,732.61;
c. Mateya Law Firm- Legal Fees for estate administration- $1,200.00
d. PA Dept. af Revenue—Estimated Inheritance Tax - $99$.30;
e. UGI Gas Service—Electric service at residence in the amount of$145.93.
f. Stardust Storage—Starage rental for decedent's personal/household effects- $120.OQ.
g. Lori Lee Jakubs—Expenses assaciated with administration of decedent's estate-
$2,830.89.
8. The Pennsylvania lnheritance Ta.x Return has nat been filed and the inheritance tax due
upon the decedent's estate has not been paid.
9. It is proposed that the following distribution af the decedent's estate be rnade to the
fallowing creditors,heirs or next of kin:
a. Hoffman Roth Funeral Home—with an amaunt due of$2,629.3$;
b. Jeffrey S. Austin—cleaning/damages at last residence$1,732.61;
c. Mateya Law Firm - Legal Fees for estate administration- $1,200.04
d. PA Dept. of Revenue—Estimated Inheritance Tax- $99$30;
e. UGI Gas Serv'rce—Electric service in the amount of$145.93;
f. Lori Lee Jakubs- $2,830.89 (expense reimbursement} and one-half of the residual estate;
g, Phill'zp.Arrigo—Une-half of the residual estate;
h. Stardust Stc�rage—Storage rental far decedent's persanallhousehold effects - $120.00.
WHEREFORE,Petitioner respectfully requests this Honorable Caurt to decree the distribution
of the decedent's personal estate to the persons entitled thereto as set forth in Paragraph 9 abave.
